Scott County Hospital Home Health provides professional health care services to
persons who are homebound. Services are coordinated on an individual basis
according to the clients needs.
Through home care, clients receive quality skilled nursing care and restorative
therapy in the comfort of their home.
Care is provided as a team approach between physician, nurses, and therapists
with flexibility to the client.
Anyone may refer a client to home health care, but services are provided only by
a physicians order.
Services Provided:
Home care services fall into 3 categories - Skilled Nursing, Home Health Aide
services, and Restorative therapy.
Registered Nurses and Licensed Practical Nurses Provide:
Physical Assessments, Client Teaching/Education, Wound Care, Blood Testing,
Injections, Preventive health measures, Rehabilitative Nursing, Foot Care for
Diabetics and others with circulatory problems, and Medication education and set
up.
Home Health Aide Services Include:
Bathing and Personal Hygiene, Meal Preparation, Exercises, Monitoring of Vital
Signs, and run errands.
Restorative Therapy Includes:
Physical Therapists/Assistants
Speech Therapists
Our professionals carry out the clients plan of care as designed by the
physician, nurse, therapist, patient and family.
Our nurses and therapists monitor and report progress to the physician. We
inform, educate and instruct individuals and their families about their illness
and assist them with obtaining needed health care services.
Durable Medical Equipment
Equipment and supplies needed by the client will be provided, or assistance in
obtaining necessary equipment or supplies is available.
Qualifying For Home Health:
1) The client must be under the care of a physician who certifies there is a
need for Home Health.
2) For Medicare reimbursement the client must be homebound where it is
physically impossible to leave home without assistance.
3) The client has a need for skilled nursing services or physical therapy on an
intermittent basis.
Payment
Home care services are paid for from a variety of sources including Medicare,
Medicaid, BC/BS, most private insurance, and occasionally by the individual.
Benefits may vary so feel free to consult our agency if you have insurance
questions.
For your convenience, our trained staff handles the filing of claims.

Our home health serves the residents of Scott County and adjacent counties
served by Scott County Hospital physicians.
Office Hours Are:
Monday thru Friday, 8:00 a.m. to 4:00 p.m. excluding major holidays. We
are located at Scott County Hospital's South Campus, 1602 South Main.
