Health Information Management
(Medical Records)
The
Health Information Management (HIM) Department is responsible for ensuring each
health record is complete, accurate, kept confidential and made available when
needed.
Department hours are Monday through Friday,
8 a.m. to 4 p.m.
Requesting Medical Records
When copies of a patient's medical record are needed, the Release of Information
(ROI) staff is available to help during regular departmental hours posted above.
To obtain a copy of medical record, call us at
620-872-7726 or mail us a completed Authorization
for Release of Health Information form (for release to a third
party such as another hospital, clinic, or insurance company)
or a completed Request
to Inspect or Copy Health Information form (for personal access.)
Please note that a fee may apply, so please take a moment to read the
information below. If you choose to download and print the authorization
form or the request to inspect form, complete it and mail or fax to:

The Health Insurance Portability and Accountability Act (HIPAA) requires that
all portions of the authorization form be completed. The form must be
signed and dated by the patient or their legal representative. Please call
620-872-7726 if you have any questions. See instructions below for
completing the appropriate forms.
How much does it cost?
Continuing Care - There
is no charge to send your information to another physician or health care
facility for continued care.
Personal Request - There
is a reproduction fee of $0.63 per page up to 250 pages and $0.45 per page for
pages 251 and above.
How long will it take to get a copy of my record?
The average turnaround time for a request is two (2)
weeks. Depending on circumstances HIPAA does give the Hospital a 30 to 60
day window to respond to personal requests for medical records. You may
come to the hospital to pick up the record, but we encourage you to call
620-872-7726 to assure its availability.
Birth Certificates
Birth certificates are not available from Scott County Hospital. To
request a birth certificate please contact the Kansas Department of Health and
Environment Office of Vital Statistics at
http://www.kdheks.gov/vital/birth.html or by calling 785-296-3253
between
8:00 a.m. and 4:00 p.m. Central Time weekdays.
