Swing Bed Program Overview


A patient may need skilled care when they have medical conditions that require daily nursing care or rehabilitation services under the supervision of licensed professionals. These services must be medically necessary and determined appropriate by a physician.

To qualify for Swing Bed services, the following criteria must be met:

The patient must have completed a 3-day inpatient hospital stay.

Admission to the Swing Bed program must occur within 30 days of discharge from an acute care facility.

The patient must require continued treatment related to the condition treated during the acute care stay.

What Is Skilled Care?

Skilled care offers 24-hour nursing supervision in a safe, comfortable environment. Patients benefit from rehabilitation, personalized activities, and supportive services, all while maintaining their personal dignity and independence. When possible, patients are encouraged to participate in their own care and work toward regaining independence. Family members are also invited to take part in the patient's daily routine and recovery process.

Our Interdisciplinary Health Care Team Includes:

Physicians, RNs, and LPNs – Provide around-the-clock skilled care.

Registered Pharmacist – Oversees medication safety and management.

Registered Dietitian – Plans and supervises nutritious meals tailored to each patient’s medical needs.

Therapists – Assist with physical rehabilitation and daily living skills.

Licensed Social Worker – Offers emotional support and discharge planning assistance.

Swing Bed Coordinator – Monitors and manages the day-to-day operations of the Swing Bed program.
